Educational settings making a referral for a medical place

To apply for a place, the following documents must be complete and submitted by a Thursday for consideration at the panel the following Tuesday.

• Referral form – submitted by the school / educational setting
• Healthcare evidence submission form - submitted by a medical professional

Schools and other educational settings will also need to have drawn upon the following in their current support for the child or young person:

• Supporting pupils with medical conditions at school - GOV.UK
• Early Help and support for families
• Advice on individual health care plans
• Information for SEND Professionals
• The Graduated Approach
• Support from Pupil and School Support
• Support from Birmingham Educational Psychology
