Education Plan for PLAC
Before the child or young person transitions out of care, there will be a final Personal Education Plan (PEP) meeting.
This meeting will focus on:
- agreeing targets
- identifying future support needs
- problem solving
- sharing information
Meeting attendance
A member of Birmingham Virtual School will attend. The PLAC Advisor may also attend.
Early Years
The Early Years Advisor will chair and take the minutes of the meeting.
Schools, colleges and other educational settings
The designated person will decide who will chair and take minutes of the meeting.
The educational setting will decide which other professional(s) need to be invited.
Parents will decide who will attend the meeting.
Education planning
Our Education Plans for Previously Looked After Children (EPACs) can be used in the final PEP meeting.
The education plan is a confidential working document to be kept in the education setting.
It is divided into different sections, to meet the needs of the young person.
