Make a referral

You can make a referral to the transition team through:

  • Children’s Services, Birmingham Children’s Trust (BCT), and Special Educational Needs and Disabilities (SEND) via the multi-agency planning meeting for young people likely to meet Care Act eligibility
  • our contact centre - you can submit a request through the contact centre – your request will be sent to the transition team or another appropriate service
