If the composition of your household changes, such as individuals moving in or out, please provide their details on this form to keep your council tax account up to date.

This form constitutes an application under the Regulation 14 of the Council Tax (Administration & Enforcement) Regulations 1992 (SI 1992 No. 613).

You must read and agree to the declaration below before submitting this form:

I commit to notifying the Revenues Manager within 21 days should the number of residents in the property change. I acknowledge that receiving a discount to which I am not entitled is a criminal offence, as is providing false information. I also verify that the information provided on the subsequent form has been distributed to all pertinent council departments, including the Register of Electors.
