Community Care Collaborative programme

Birmingham’s Adult Social Care team is working in partnership with social care, primary care, community health services, community mental health services, and community and voluntary sector organisations across the city to provide better and more joined-up care for citizens at a neighbourhood level.

Together we are all working to deliver the Community Care Collaborative programme. This aims to:

  • provide better joined-up health and care services in local neighbourhoods across Birmingham
  • make it easier for people to get the care they need, when and where they need it, from the right provider
  • help people stay healthy and independent in their own homes for longer

The  Birmingham and Solihull Integrated Care System oversees the implementation of the Community Care Collaborative, with the Birmingham Community Healthcare NHS Foundation Trust leading the initiative, alongside Birmingham Adult Social Care as one of the partners involved.

What does this mean for Adult Social Care?

For Adult Social Care, this initiative helps us support people to stay healthy and independent in their own homes and communities for longer.

What does it mean for me as a Birmingham citizen?

We want our citizens to live healthy, resilient and independent lives, with simple access to support and care when needed, as close to home as possible.

Across Birmingham health and social care teams are working more closely together, often from the same building, to make sure that citizens across Birmingham get the care they need when they need it and by the right person.

These teams include GPs, district nursing and therapy colleagues, mental health professionals, voluntary sector, social care and other council services.

Their services could include social prescribing, medication reviews, healthy lifestyle advice and support, community mental health support and support for carers and families. They also have strong links into voluntary organisations, care homes, end-of-life care and hospices and community network teams.

The aim is to deliver more joined-up preventative care in your community. By sharing resources and information, the teams can work better together, getting you the services you need.

You may have already been contacted by a local integrated neighbourhood team or benefited from virtual wards, the early intervention community team or the urgent community response team.  All these services are part of the Community Care Collaborative.
