Education plan for PLAC

The Education Plan is an optional form that can be used at a meeting for Adopted Children (EPAC/ AO/SGO).

It can be completed with consultation by Parents or Guardians with support from the Designated Person at the educational setting.

This meeting (EPAC/ SGO/AO) should be attended by professionals who parents/guardians choose to invite.

It is a confidential meeting, where positive collaboration and communication is discussed, to raise awareness of the needs and issues, which are faced by the young people and parents/guardians.

These meetings are a useful way of sharing information, problem solving and setting targets for the future.

Who organises the meetings?

In Early Years setting the meeting is chaired, organised and minutes taken by the Early Years Manager/Designated Person, who is linked to the setting.

In other Educational Settings parents and the Designated Person decides who chairs and minutes the meeting.
