Reduction for Disabled Facilities

The information that you provide on this form will be used to maintain Council Tax and any related Benefit records. The information may be compared with that held by other Council departments, or other organisations, for the purposes of preventing or detecting fraud and in accordance with the Authority's notification under the Data Protection Act 1998.

You should fill in this form if you wish to claim money off your Council Tax bill because your home has been adapted for a disabled person.

You must notify the Council immediately if you believe you are no longer entitled to a reduction granted in respect of this application.
