Four offences relating to conditions at Caribbean Flavaz Ltd, 32-34 Holloway Circus Queensway, Birmingham. There were no permanent procedures based on HACCP principles. There was mouse activity at the premises and gaps in the structure that could permit the ingress of mice. Floors, freezers, shelving, a wall, a staircase, pipework and a work surface were dirty. Flooring was damaged, a kitchen sink was leaking and metal work beneath the staircase was rusting.
