Report a lost or stolen licence

Have you lost your badge or plate or has it been stolen? Then you need to report it to us.

Please complete the report lost/stolen licence (badge/plate) form, via the link below, and submit your completed form to licensing@birmingham.gov.uk. You must include in the subject line of your email what the form is for e.g. report stolen HC plate

Please note applications will NOT be accepted unless all parts of the form are completed in full, and the relevant documentation provided. There will be a short waiting time in order to process your request.

We are aware this application form may not be suitable for users of assistive technology. Please email licensing@birmingham.gov.uk if you require an alternative format.
