Be Active Plus

Be active Plus is a GP referral scheme for patients with specific medical conditions that can benefit from structured exercise programmes.

The scheme involves a GP or health professional referring a patient to see a Health & Fitness Advisor at a city council leisure facility. People are referred to be more active to help them to improve any long term conditions they have. These conditions can include coronary heart disease, stroke, COPD, diabetes and some mental health conditions. The advisor carries out an initial consultation and a 12 week exercise plan is agreed. Patients take part in the sessions and are supported by their advisor through follow-up consultations.

Northfield Main Pool

It is funded by NHS Heart of Birmingham, NHS Birmingham East and North and NHS South Birmingham Primary Care Trusts

For six months from October 2011 there is a pilot scheme in partnership with Birmingham & Solihull Mental Health Foundation Trust. An additional health & fitness advisor has been seconded from the Trust to provide Be Active Plus for patients who visit the mental health clinics at Soho Health Centre and the Zinnia Centre.
