What is in the statement?
The SEN Statement contains the following information:
Part One:general identity information about your child and you as the parents / carers: name, address, date of birth, language and religion.
Part Two:details of the special educational needs identified during the assessment process.
Part Three:description of all special educational support that needs to be provided for your child.
Part Four:gives the name and type of school that your child should attend to receive the support described in Part Three.
Part Five:describes any other (non-educational) needs for which the LA considers appropriate if the child is to benefit from provision set out in Part Three.
Part Six:describes the provision required to meet non-educational needs set out in Part Five, as agreed with health and / or social services and the LA.
