What is person centred planning?
Person centred planning is a way of supporting you to make choices and changes in your life.
It is a way of helping you to plan your future and organise any support and services you may need.
We use person centred planning to help you deal with changes that you may not have been able to make by yourself.
What happens in person centred planning? |

Person centred planning usually starts with a meeting, to help you to find out about the things in your life that need to change.
You choose who you want at the meeting.

At the meeting, the facilitator will use different person centred planning ways of working to help get information.
|

|
When we have gathered all the information, we work with you to make a plan of action. This is about how you choose to be supported to make changes in your life.

Who is person centred planning for?Anyone can use person centred planning to help make changes in their life. We use person centred planning in our work with adults in Birmingham who have a learning disability.
