Be Active Plus

Be active plus is a GP referral exercise programme for people with specific medical conditions that can benefit from being more active.

Briefly these conditions include:

  • Coronary heart disease, including previous cardiac event (e.g. a heart attack or heart surgery) over 6 months ago
  • High blood pressure (no higher than 160/100)
  • Obesity (a Body Mass Index of over 30)
  • Diabetes
  • Asthma
  • Stroke or TIA over 12 months ago
  • Neurological conditions e.g. Parkinson’s, stable MS
  • Depression and / or anxiety
  • Arthritis
  • COPD (Chronic Obstructive Pulmonary Disease)

Getting Started

To join the Be active plus scheme a person needs to be referred by their GP or Practice Nurse at their surgery. The staff at the surgery will check that the scheme is suitable for them and then complete a referral form. Once the details from the form have been submitted to the Be active team the person will receive a call (or a letter) from a Health and Fitness Advisor at a local leisure centre. They will invite the person in for a first appointment at a convenient time.

The scheme is funded by Birmingham Public Health, and so all GP surgeries in Birmingham are able to refer patients.

These are the 10 leisure centres where a Health and Fitness Advisor has an office. It is possible to arrange to have appointments at other city council leisure centres and to exercise in many other venues across the city.

What happens on the scheme?

The first appointment is for the person to find out more about the scheme and for the Health and Fitness Advisor to find out about what sort of activities the person might like to try. An individually tailored exercise programme will be written for them – taking into account their current fitness level, medical conditions, activity likes and dislikes and availability. They will be supported during their 12 weeks on the scheme. At the end of 12 weeks the person will have an exit appointment to discuss their progress and plan their future exercise programme. A report is sent back to their GP. They will also be contacted 3 and 6 months after they have finished on the scheme to see if they are still exercising.

How much does it cost?

It does not cost participants to be on the Be active scheme, but they would be charged the admission rates of any activities they attend. The cost of this depends on which activity a person chooses to do, at which leisure centre, whether they have a Passport to Leisure or pay by direct debit etc. All costs and any possible reductions in costs will be explained by the Health and Fitness Advisor at the first appointment. Participants will be encouraged to join Be active scheme which provides many activities across the city free of charge.
