Community Links Service

The Community Links service can help you find out about and become involved in activities and opportunities in your local area.
This can include activities or events that may be taking place during the day, in the evenings or at weekends. A Community Links service is based within each of our new care centres and will give support to both individuals who live in the care centre and to those in the local community.

The Community Links service will:

  • Be personal to you. We will work with you on a one-to-one basis using person centred approaches to help you become involved in activities in the community or other opportunities. These could include leisure activities, education or social contact. A person centred approach puts you at the centre of any involvement you may choose to have and supports you to make decisions and live the life you choose.
  • Promote activities, sessions and events within the care centre and the local community for residents of the centre or people in the wider community.
  • Provide information within the care centres about citywide and community resources..
  • Work with other groups and organisations to develop and support community links.

Who can get this service?
To take part in this service you will need to be over 65 and be referred to our service. We will work with people who live in the care centre, in the constituency served by the care centre or people who access some of its groups.
Depending on your circumstances you may be referred to us by a social worker or your care centre key worker.

Finding out what you want
When you have been referred to the service a member of the team will visit you. This will help us to get to know you, find out what things you like to do or may be interested in doing. We will also find out what you do at the moment and who is important to you.

We will then work with you to write down this information and put it into a plan showing how you can reach your goals. We will work with you to research, find out about and create any links and networks to achieve your goals. If you have been referred by a Social worker we will provide you with support for up to 8 weeks. This support will be to help you become as independent as possible in any opportunity or activity you choose with the aim of developing your social contacts, relationships and ultimately your quality of life.

About the activities
Activities will take place in local community centres, libraries, leisure facilities and other places of your choice, and will be when you choose.

Travel
We will support you to use a variety of means of travel to get to an activity. This will depend on the activity, your individual circumstances and finances. This may include Ring and Ride or public transport. You will need to fund this and any activity yourself during the time we are supporting you.

Care centre activities and resources
We will promote and organise activities and sessions for older people within the care centre. We will make sure that these reflect the needs of older people in the community. We will do this by working with local organisations and older people to arrange a variety of activities and groups for older people to attend as they wish during the day, evenings and weekends.
If you are an older person living in the local area served by your care centre we will have a range of resources for you to use when you wish, such as information areas and internet access. So, feel free to visit the centre to see what's on offer and how you can get involved.

We value your opinions. If you have any ideas or suggestions of what you would like to see happening in the centre please feel free to contact us or speak to one of the workers at the centre. We also want to hear from you if you are an individual, group or organisation providing support to older people and would like to provide an activity or session.
